ABSTRACT

Many complicated bone fractures can be healed by different techniques including linear external fixation. New generation linear external fixators enhanced the scope of application for bone fractures by mechanical progress. Difficulties include implementing enough pins through fixation clamps to the comminuted fragments or clamp addition and fixing it properly to the rod with the correct pin insertion angle. Effective configurations may not always result. This study sought to reveal the clinical efficiency of modified clamps of a versatile external fixator (VEF) to fix different types of fractures and orthopedic problems according to the radiographic and clinical results. We used this fixator on 17 cats and 17 dogs of different ages and sizes, having different types of antebrachium, humerus, tibia, ulna fractures, and bone-muscle deformities. Clamps had different features to connect fixator pins. Many fixator configurations were created according to the fracture type and body weight of the animals. The most used ones were unilateral and bilateral configurations. The callus formation and visual gait analysis were observed after the operations, until the removal of the fixator. After fixator removal, the visual gait status of the limbs was excellent in 67% of the cases, good in 15%, fair in 12%, and poor in 6%. We found that rods and fixator pins were connected easily by semi-locked clamps. Also, the double pin holding clamps saved space on the fixation rod by the application of two pins through one clamp. We think that clamps of versatile external fixators can easily be constructed to limb fractures and save time during surgery.(AU)


Diversas fraturas ósseas complicadas podem ser tratadas por diferentes técnicas, incluindo fixadores externos lineares. Os fixadores externos lineares de nova geração aumentaram o escopo de aplicação para fraturas ósseas devido ao progresso mecânico. A dificuldade é implementar um número suficiente de pinos através de clamps de fixação aos fragmentos osseos ou adição de clamps com fixação adequadamente à haste com o ângulo correto de inserção do pino. As configurações eficazes nem sempre podem ser realizadas. Este estudo pretendia revelar a eficácia clínica das clamps modificadas de um fixador externo versátil para corrigir diferentes tipos de fraturas e problemas ortopédicos de acordo com os resultados radiográficos e clínicos. O fixador foi utilizado em 17 gatos e 17 cães de diferentes idades e tamanhos, com diferentes tipos de antebraço, úmero, tíbia, fratura de ulna e deformidades ósseo-musculares. Os clamps tinham recursos diferentes para conectar os pinos do fixador. Muitas configurações de fixadores foram criadas de acordo com o tipo de fratura e peso corporal dos animais. As mais utilizadas foram as configurações unilateral e bilateral. A formação do calo e a análise visual da marcha foram observadas após as cirurgias, até a retirada do fixador. Após a retirada do fixador, a utilização dos membros foi excelente em 67% dos casos, bom em 15% dos casos, regular em 12% dos casos, ruim em 6% dos casos. Concluímos que as hastes e os pinos do fixador foram facilmente conectados por clamps semi-travados. Além disso, os clamps de fixação de pino duplo economizaram espaço na haste de fixação com a aplicação de dois pinos em clamp. Acreditamos que os clamps de fixadores externos versáteis podem ser facilmente construídas para fraturas de membros, economizando tempo na cirurgia.(AU)

ABSTRACT

Objective: To investigate the effects of Paeoniae Alba Radix extracts (PAREs) on 5-HT3R mediated ion channels in primarily cultured hippocampal neurons of rats with depression. Methods: An animal model of depression was successfully developed and evaluated in rats. PARE was used for drug intervention. Serum in each group was collected, inactivated and then added into the primary hippocampal neurons for 24 h. The protein expression levels of 5-HT3AR and 5-HT3BR in the neurons of each group were examined by Western blotting (WB). The 5-HT3R channel current was recorded by a whole-cell patch clamp. Results: Compared with normal rats, the rats with depression had significantly reduction in total distance of the open-field test (OFT) and sucrose preference ratio (P < 0.01). The hippocampal neurons treated with serum of depressive rats had significantly increased protein expression of 5-HT3AR and 5-HT3BR (P < 0.05) and current density value (P < 0.05) compared to those treated with normal rat serum. Compared with the depressive rats, the rats treated with PARE and fluoxetine had significantly increased OFT (P < 0.05) and sucrose preference ratio (P < 0.01). The primary hippocampal neurons cultured with serum from PARE and fluoxetine-treated rats had significantly reduction in protein expression of 5-HT3AR and 5-HT3BR (P < 0.05, 0.01, 0.001) and current density value (P < 0.01). Conclusion: PARE can reduce the 5-HT3R ion channel current density in the rats with depression. This may be its central mechanism in treating depression.

Objective To investigate some points in evaluating the effects of insulin sensitizer on obesityassociated non-insulin-dependent diabetes mellitus Zucker fa/fa rat model. Methods Total 20 male Zucker fa/fa rats were divided into model group and rosiglitazone groups. Another 10 male Zucker fa/? rats served as normal control. Rosiglitazone at 6 mg/kg was given intragastrically once per day for 4 weeks,and the rats of the other 2 groups were fed with same solvent at same volume. ABSTRACT

Background and purpose:VATS lobectomy has gained popularity for lung cancer around the world.Complete anatomic resections and node dissections are routinely being performed at many centers under VATS,but some thoracic surgeons are concerned regarding the safety,benefit and radical resection of this operative method.Our study is to evaluate the reliability and feasibility of pulmonary resection by video-assisted mini-thoractomy(VAMT) for patients with lung cancer.Methods:72 patients with lung cancer received either lobectomy and systemic lymph node dissection by video-assisted mini-thoracotomy(VAMT group;n=32) or conventional thoracotomy(conventional group;n=40),lobectomy was performed by conventional or thoracoscopic instruments,specific lymph node clamps were applied for lymph node dissection.Results:VAMT was successfully performed without significant postoperative complication and blood transfusion.No significant differences were observed in the two groups with respect to the length of operation and the total groups of dissected lymph nodes.This study showed that VAMT is a minimally invasive surgery without impairing of the outcome and needed less operative blood loss and shorter postoperative hospitalization compared to conventional thoracotomy.Conclusions:Major pulmonary resection by video-assisted minithoractomy for patients with lung cancer is safe,reliable and less invasive,it is consistant with the surgical standard of lung cancer.Its long-term benefit needs to be clarified after further follow-up.

ABSTRACT

Objective To evaluate the security and the reliability of ligating the cystic duct and the cystic ar- tery with common silk suture as the substitute for titanic or biological clamps in laparoscopic cholecystectomy (LC) us- ing the suture and knot instrument developed by the authors. Methods One hundred and two laparoscopic chole- cystectomies were performed by the authors using the instrument. Results All patients were discharged in the post- operative fourth or fifth day without any complication. Conclusion The instrument used for LC was safe and reliable.

ABSTRACT

Conventional methods for stabilization of unstable cervical spines caused by flexion injury are sublaminar wiring and palte and screw fixation. These methods, however, carry the risk of damage to the cervical cord and or vertebral artery. We have used interlaminar clamps for the stabilization of sixteen cases of unstable cervical spine, including for cases of odontoid process fracture. There was suecessful fusion in all cases, no slippage of clamp and no deterioration of neurological function. Advantages of this method include the fact the there is no need for bone graft and that it is technically easy and inexpensive. Good stabilization allows early ambulation and so prevents complications such as pneumonia and decubitus ulcer.
